Abstract
A 25-year-old female with a medical history of congenital dyserythropoietic anemia type I (average of hemoglobin level was
8 gr.) and mild iron overload, presented to the emergency department due to a history of abdominal bloating and a 1-week of

abdominal pain especially in left upper quadrant. During physical examination was noted tenderness and a mass in the midabdominal area. A computed tomography (CT) scan was performed and showed torsion of wandering spleen (splenomegaly
17 × 13 cm that pushes the intestinal loops (red square)), ischemic areas in the spleen and splenic vein thrombosis. The
patient underwent an urgent splenectomy (also she was treated with anti-pneumococcal vaccine (PNEUMOVAX)). As
known that hemolytic anemia increasing the risk for thrombotic manifestations (venous thromboembolism), therefore since
the surgery the patient is on anticoagulation therapy.
